Cancellation Policy

Monthly membership service is offered as a cost-effective approach to maintaining regular neurotoxin injections for treatment of facial lines and wrinkles. Patient has been evaluated at Welcome Health & Aesthetics by nurse practitioner and is deemed appropriate and eligible for minimum 40 units of cosmetic neurotoxin treatment, per recommendations for use by Allergan (Botox® Cosmetic) or Evolus (Jeuveau®). Patient understands some treatment areas may not be FDA-approved and therefore considered "off label" use. Monthly membership entitles the above-named patient to neurotoxin treatment of 40-50 units every 90-120 days, depending on plan selected. If patient desires less than 40 units at the time of treatment, units are not banked or rolled over for future use. If 51 units or more are desired and deemed appropriate at a single treatment, then additional units of Botox® or Jeuveau® will be purchased at the rate of $12.75 per unit. Patient is guaranteed the prepaid rate for units of neurotoxin while participating in the monthly membership plan, even if regular cost increases. Patient has read and signed the Botox® Cosmetic/Jeuveau® Consent for Treatment. Patient understands monthly payments will be invoiced and must be paid prior to administration of neurotoxin treatment in order to receive the discounted rate. Invoice will be sent at the beginning of each month and due upon receipt. Payment date does not adjust the 90-day treatment window. Patient agrees to notify Welcome Health & Aesthetics in writing, 30 days in advance of next payment, if patient wishes to discontinue participation in membership plan, and will forego discounted pricing for future treatments. Patient can send cancellation via email (mywelcomehealth@gmail.com), appointment booking messaging (Square), or EHR message service (Tebra). If less than 30-days notice has been provided, patient may receive a monthly invoice. If membership is cancelled and less than 3 payments have been prepaid, then the unused prepayment(s) (not already applied to previous treatments) will be applied toward a neurotoxin treatment at the regular rate (currently $14 per unit). Patient may gift one prepaid cosmetic neurotoxin treatment to a family member or friend during a 12-month membership period. Any patient receiving a gifted treatment must be evaluated and eligible for neurotoxin treatment and must sign consents for treatment. Additional consent forms to be signed prior to treatment.
