Service Description
We will review your personal journey and goals for health and wellness. Includes labs and 30-day supply of single medication. Additional medications may be prescribed with ongoing care. Whether you are starting or continuing your journey as a transperson, we offer a safe and welcoming space to address your healthcare needs for gender affirming hormone replacement.
Cancellation Policy
WELCOME HEALTH LLC, dba Welcome Health & Aesthetics Services Client in receipt of this agreement has contacted Welcome Health, LLC, (dba Welcome Health & Aesthetics) for the purpose of initiating services, including but not limited to: weight management, hormone therapy, medical aesthetics (Botox or Jeuveau, Juvederm filler, chemical peel). Client agrees to pay a deposit in the amount of 10% of service at the time of booking, which will be applied to service or treatment. If the client reschedules less than 24 hours prior to, cancels less than 48 hours prior to, or "no-shows" the scheduled appointment, the deposit will be forfeited. Payment at the time of services Welcome Health, LLC (dba Welcome Health & Aesthetics) will bill the Client and requires payment, less any deposit paid, at the time of services. The Client agrees to pay in full for office visit, labs, and special order compounded medications at the time of consultation, and development and agreement of plan of care, and ordering of labs or medications. Monthly medication supply will be ordered after receipt of payment.
